BC Medical Services Plan Travel Assistance Program (TAP) forms need to be provided by the <u>referring physician</u> before the date of your travel. Therefore if you are traveling to the Helmcken Pain Clinic on a TAP form your referring doctor must complete the form for you. Many FP offices can <u>mail</u> the form to you in advance of your appointment. The Helmcken Pain Clinic only provides these forms when we refer you to someone else outside our clinic.
